Complete and submit the forms below (1-4) before your scheduled appointment time.

Please call Laurel Gonzales, Billing Specialist, at 512-292-1122 (at your earliest convenience) and leave a message stating that you have ab appointment with Dr. Cardona. Please include your contact information, insurance information and date of birth. Laurel will let Dr. Cardona know if you have a deductible as well as your copay amount. 

Instead of calling Laurel you may upload your insurance card and Photo Id or email them to lauracardona@myaustinpsychologyclinic.com.

Please note that you will be responsible for the cost of therapy if I have not heard back from Laurel by the time the appointment takes place. This fee will be adjusted once your insurance claim has been processed and, if appropriate, you will receive a refund.
